Step 1.\u00a0<\/span><\/b>Start a New Referral: Access the Form<\/span><\/b>\u00a0<\/span><\/p>\n

1.1 From\u00a0the Home screen, navigate to the\u00a0\u2018Referrals\u2019\u00a0section<\/span>\u00a0<\/span><\/p>\n

1.2. Locate and open the\u00a0\u2018Referral Form\u2019 to\u00a0submit\u00a0a new\u00a0member\u00a0referral<\/span>\u00a0<\/span><\/p>\n

2.2 Enter Medi-Cal\u00a0Number.\u00a0<\/span>If the member\u2019s Medi-Cal Number is available, enter it in the Medi-Cal Number field. If not, enter the Social Security Number (SSN). If the Medi-Cal Number is not available, the SSN, email ID, or contact number will be used as the MCIN for the member.<\/span>\u00a0<\/span><\/p>\n

2.3 Choose\u00a0a main requested service\u00a0for the members if\u00a0required\u00a0(e.g.,\u00a024×7 Assisted Living,\u00a0Aging,\u00a0Birth\u00a0Equity, CHW\u00a0etc.).<\/span>\u00a0<\/span><\/p>\n
